Overview
At the heart of Clinication is the CareGuide™, the patient-specific, highly
articulated plan-of-care. The CareGuide is comprised of the seven key building
blocks of care requirements, namely Medications, Exercises, Procedures of Self
Care, Scheduled Appointments, Data Collection Inquiries, General Instructions
(disease information, nutrition) and Equipment Usage Instructions. This innovative
model facilitates utilizing Clinication to manage virtually any type of disease
or condition.
Clinication includes an extensive library of Care Elements (over
100,000 medications!) but providers are free to add their own "favorite" care
elements to the system.
Library elements include valuable patient information written at an appropriate
level, in multiple languages. Medication information includes drug images for
ease of identification by patients and caregivers.
Once a Patient CareGuide is published by the provider, typically
a 30-60 second process, it is securely available online for the patient and
caregiver, who are encouraged to access the system daily and report back both
adherence to prescribed Care Elements, and respond to the data collection Inquiries,
which represent a powerful, yet infinitely flexible method of collecting virtually
any type of information from the patient, from daily vitals through pain scales,
self-assessment questionnaires and satisfaction surveys. Based on predetermined
rules, Clinication will alert the appropriate provider in case of variances,
including non reporting, noncompliance, and out-of-range values.
The patient may benefit from automated prompts and reminders
which message directly to e-Mail, digital pagers, or SMS messages on cell phones. A
web-based, encrypted messaging system with rules-based engine gives patients
and caregivers the maximum flexibility in commonucations, while assuring providers
of appropriate automated message triage and context-sensitive routing.
